2019 SYC FOOTBALL SEASON INFORMATION

 

Required Participation Forms:

 

1.    Birth Certificate (required upload)

2.    Proof of Residency  

   a.    Current 2019-2020 Southern York County School District report card  

   b.    Southern York County School District Bus Stop Information

   c.    In the event that the above proofs of residency are not available 2 documents must be submitted from the following categories to determine residency of parent/guardian: (parent/guardian’s last names must coincide with the last name of player and name/address on both documents)

                                         i.    Driver’s License

                                        ii.    Voter’s Registration

                                       iii.    Utility Bills (Cable; gas; electric; water/sewer)

3.    Medical Release form for player in Smurf or Rink division
       (Please complete this form if you are a parent/guardian of a child participating in SMURF

       or RINK Divisions only)

4.    Medical Release form for player in Pony or Midget division
       (Please fill out this form if you are a parent/guardian of a child participating in PONY or

       MIDGET Divisions only)

Players CANNOT pick up their equipment until ALL forms are signed and uploaded.
