REQUIRED FORMS FOR PARTICIPATION

In order to participate in athletics in Montgomery County Public Schools, there are many forms that must be completed in advance of being allowed to step onto the field, even for a tryout.  New for 2018 is an online process for submitting these forms through an MCPS portal.  Please visit the Wootton Athletics website (www.woottonathletics.com) for more information and the link to the online portal.  If you choose to print out the summary of those forms for informational purposes, be aware that it is an extremely long printout!  One note:  the document suggests that you must complete the Booster Club Membership forms in advance of tryouts, but these two forms only need to be completed if you are selected to the team.

Here are some other forms you will need to submit, if selected for the team.

The Driver Rider Form is required in order to allow your child to participate in parent-driven car pools.  If you approve your child's participation in those car pools, you should indicate that they are allowed to be driven by the "parent of any Wootton Soccer player" or by "school-provided transportation", such as a school bus.  However, nearly all "away" games will involve parent-driven car pool, and individual players are NOT allowed to drive themselves or ride with a teammate to an away game (these are school rules).

All parents are strongly encouraged to complete the Wootton Booster Club Membership Form and join the Wootton Booster Club.  This is an annual membership, and covers all extracurricular activities for your child, so if he participates in a winter or spring activity, you will not be asked to "join again".  The Booster Club supports equipment purchases, the weight room, the concession stand, team banquets, and other activities throughout the school year.
