GCYC SPORTS PHYSICAL EXAM REQUIREMENTS
ALL participants from Football or Cheer must have a GCYC Physical form filled out by a medical professional prior to participation. The exam date for the physical form must occur in the SAME CALENDAR year as the season of play. For example: For the 2024 season the exam date on the physical form must have occurred on or after January 1, 2024. Click on the Physical Form's name to download the file.
