Locker Soccer Elite Player/Coach Forms

ALL PLAYERS WILL NEED A COPY OF THEIR BIRTH CERTIFICATE AND TWO (2), 1" X 1" COLOR PHOTOS. Forms can be turned in at Locker Soccer Academy in Powell. We will also hold a formal Paperwork & Uniform fitting meeting in late June.
