Maywood Youth Athletic Association Volunteer Coaching Requirements Checklist
Thank you for your interest in volunteer coaching for the Maywood Youth Athletic Association.
The following is a checklist that ALL NEW COACHES MUST COMPLETE before holding any practices or coaching any games:
1. Register for and attend a scheduled Rutgers SAFETY Clinic Course (3 hours):
· This course protects volunteer coaches from civil lawsuits under the “Little League Law”.
· An online course opportunity will be provided by MYAA representatives and will provide you with information that is crucial to the success of your volunteerism.

2. Complete the Heads-Up Concussion Training:
· This self-paced online training session will allow you to provide proactive and reactive care for your athletes in the event of a head injury. With youth sports research proving the dangers of untreated concussions, this training is necessary for effective volunteer coaching. The link for the training session is below:

3. Register and complete fingerprinting and background check process:
· Visit https://www.identogo.com/locations/new-jersey to register for fingerprinting.
· You will make an appointment to visit Identogo in Garfield, NJ.
· During the registration process you will be prompted to provide a Service Code and Contributor Case Number. For MYAA volunteer coaching please provide the following two codes:
Service Code: 2F1J3Y
Contributor Case Number: B13001
· The visit is seamless, quick, and imperative to your volunteer coaching process.
· All returning volunteer coaches must complete this process every three years.
REMINDER: NO VOLUNTEER COACH WITH AN INCOMPLETE CHECKLIST MAY PARTICIPATE IN ANY PRACTICES OR GAMES.
