2020 BROOMFIELD SHOOTOUT CHECK IN INSTRUCTIONS
Option 1: CHECK IN ONLINE
Deadline: Friday, August 7th, 2020 by 5:00 PM (Mountain Time
Teams that complete online check in will receive a confirmation email by Friday, August 2nd, 2019 at 7:00 p.m. Confirmation email will be sent to the email address as a reply to the email from which the check in documents are sent.
Directions to Complete Online Check In:
1. Fill out the “2020 Broomfield Shootout Team Information Sheet” (link at bottom of this page)
2. Have the following documents ready to submit:
- One copy of your State Association, League, or US Club approved roster
- Current (2020-2021 season, NOT 2019-2020 season) player passes for all members of the team (including guest players)
- Medical release forms for all members of the team (including guest players) -OR- complete the Medical Release Waiver form
- Guest player’s full name, player identification number and date of birth at the bottom of the existing approved roster
- Permission to Travel form, IF OUTSIDE OF REGION 4 (SEE FREQUENTLY ASKED QUESTIONS)
3. Submission process:
- SCAN each document.
- SAVE each document type as an individual PDF as they will be uploaded separately.
- Please use descriptive titles so you will know which files to upload.
- Please upload all player cards as one PDF document.
- LOG IN to the gotsoccer team account which was used to register your team, click on "2020 Broomfield Shootout", click on "Documents", and upload each of the required items with the correct label. After uploading all documents, click on the "Notify" button (no need to add a message).
ROSTER UPDATES (If applicable):
- If a roster update is required after online check in has been completed, a team representative MUST attend physical check in to update the roster. This includes the addition of guest players not included on the roster already submitted for online check in.

Frequently Asked Questions
How do I add a guest player?
Step 1: OBTAIN the guest players card and medical release form.
Step 2: ADD the guest player by handwriting full name, player identification number, and date of birth to the bottom of the existing approved roster.
Step 3: SCAN and COMBINE into a PDF document (online only).
Can I have different player passes from different organizations?
Mixed rosters will NOT be allowed – NO exceptions. US Club teams may only take a guest player registered under US Club Soccer. USYS teams may only take guest players registered with properly stamped USYS player pass cards.
What is the permission to travel form, and do I need it?
Permission to travel forms are NOT required for teams in Region 4, which includes the following states:
Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, Nevada, New Mexico, Oregon, Utah, Washington, Wyoming
What if I cannot personally make it to physical check-in?
Any representative of the team (coach, assistant coach, manager, or parent) may complete the check-in process. We strongly recommend completing the online check in process at it will save you considerable time.
What if my team does not complete check in?
Failure to complete check in will result in all games being forfeited and no refund being issued. NO EXCEPTIONS
