  Registration 1:

  May 14 from 7-11am

  Registration 2:

  May 18 from 6-8pm

  Hy-Vee

  2700 DeKalb Ave., Sycamore

   

  Registration 3:

  May 21 from 11am-1pm

  Hy-Vee

  2700 DeKalb Ave., Sycamore

   

  Cost- Ages 4-18:

  $65 per player – Fee** includes a jersey,

  AYSO membership, PLAYSOCCER Magazine,

  and supplemental accident insurance.

  Fee covers Fall season!

   

  **“Early Bird” price is $65! Regular fees after May 22^nd is $120. Players must be 3 years old by July 31st, 2016.

• Fee is $125. Mail one copy of registration form, concussion form, payment and copy of birth certificate (1st time players only) to:  AYSO, Box 209, Dekalb, IL 60115.  We ALWAYS need volunteers!
  
• Payment can be made by cash or check.
• Please make checks payable to "DeKalb AYSO".
• Receipts will be given at the time of payment.

• Returned Checks - all registration fees and any fees incurred by the Region as a result of the returned check must be paid before a child(ren) can be registered.
