INSURANCE  INFO

If a player is injured during a game at Roy Gayle Complex or at a practice, and medical care has been rendered, the parent should: 

1. Submit the claim form to their own insurance carrier. Insurance coverage provided by Roy Gayle Pony
     Baseball/Softball is a secondary provider. 

2. Notify Arthur J. Gallagher & Co. of the accident or injury within 48 hours. 

3. Complete any forms sent from Arthur J. Gallagher & Co. (broker). 

4. If a signature of a Roy Gayle Pony official is necessary, please have a member of the Executive Committee   
    sign the complet­ed form BEFORE the claim is submitted. 

The board has contracted Arthur J. Gallagher & Co. to broker our insurance. Insurance is in force only at games played at Roy Gayle Complex or practices held by manager and/or coach­es. For coverage to be effective for players during games played away from the Roy Gayle Complex, the team manager must have approval in writing by an officer of the Executive Board. 

THERE IS A $50 DEDUCTIBLE.
