| Medical Release: | | All players must turn in a signed NCYSA Medical Release form each year.

| | Player Photo: | | If your players photo was not taken at Tryouts, photos will be taken on registration night. Otherwise, please see these instructions for taking your own photo.

| | Birth Certificate: | | NEW THIS YEAR: you will need to upload this to your account. Click the “pencil” icon next to player’s name – under DOB, click the “Upload Birth Certificate” button – upload the doc – click UPDATE at the bottom of the screen to SAVE the upload (otherwise the upload will disappear). If you have trouble uploading the document, you can bring a copy to registration night.

| | Financial Aid: | | Application are due by June 13th.
Complete the Financial Aid Application and attach all required documentation.

| | Uniforms: | | Samples of our Nike uniform will be available to try on. Uniform Cycle ends in 2018. New to BSC players will need to order the full uniform kids. Returning players may order additional pieces if needed. An email will be sent out and the end of June with instructions for ordering your Nike uniform through Eurpsport. |
