2020 Fall Soccer Registration Information

Walk-in Registration

Online Registration

• One (1) digital copy (ie: scanned) of State-Certified Certificate of Live Birth.  Note that hospital or baptismal certificates are not legal documents and cannot be accepted.  California Youth Soccer Association regulations require a certificate of live birth for registration.
• One (1) digital photo of the player that shows only the head/face without any hats or sunglasses.
• A credit or debit card to pay registration fees.

Mail-in Registration

• One (1) copy of a State-Certified Certificate of Live Birth (non-returnable).  Note that hospital or baptismal certificates are not legal documents and cannot be accepted.  California Youth Soccer Association regulations require a certificate of live birth for registration.
• Two (2) completed copies of the CYSA registration form.  Note that incomplete registrations will not be accepted.
• One (1) completed copy of the OYSC Team Preference/Volunteer form.
• One (1) small un-laminated photo of the player that is approximately 1" x 1" and shows only the head/face without any hats or sunglasses.
• One (1) signed copy of the CYSA Parents Concussion Fact Sheet.
• One (1) signed check or money order payable to Orangevale Youth Soccer League.
