AYSO has now mandated all volunteers and players be registered through www.eayso.org. We encourage all families to do this process at home, the National office, 866-588-2976, is at your disposal Monday - Friday 9 am - 5 pm PST, and 9 am - 2 pm Saturdays. You must have an email address to establish an account in eayso. After July 1, mail to PO Box 66, Lahaina, HI 96767. Player's are not officially registered until properly completed forms with payment are turned in. Space can be limited, if your child cannot be placed on a team, your payment will be returned.
Age Guide for 2016-2017 Membership Year
Players must play in their designated age groups! * U10 player's practice and play in Napili Park. All other player's practice and play in Lahaina. U10-U12-U14 travel for some games.

AYSO General Release
AYSO does not encourage children to walk or ride a bicycle home or to a friend or relative’s house. However, we recognize that it may be necessary in some cases. When it is, parents/guardians should provide the
AYSO General Release Form to the coach.
AYSO Participation Release Form
Whenever a player or youth volunteer has suffered an injury or illness that required the care of a physician or a visit to an emergency care facility, the AYSO Participation Release Form must be completed in full and signed by the child’s parents or guardians before the player or youth volunteer will be allowed to participate further in the program.
This form protects not only AYSO, but more importantly, the participants. Too often participants are out playing again before they are physically ready. Players should not participate until they are ready to play again. This form should also be used in cases of major illness. The
AYSO Participation Release Form should be completed and given to the team coach.
AYSO Refund Policy
All requests for refunds must be submitted in writing to the Regional Registrar or the Regional Commissioner and show cause for the request. The Regional Board of Directors will review each case and decide on the appropriate action as a committee. This process may take several weeks.
As a general rule the following guideline will apply:
- Full refunds will be issued if;
a. The player has moved out of the Region prior to the start of the season.
b. The player is unable to play due to an injury or illness prior to the start of the season.
c. The player does not want to play and the request for a refund is submitted before August 1st.
- The current National fee may be retained if player has actively participated on the soccer field.
- Uniform cost may be retained if not returned.
