NEW REFEREE SIGN-UP/TRAINING


To become a certified referee, please perform the following 3 steps:


1. Go to https://www.eayso.org and register as an adult/youth volunteer.


     a. If this is the first time you register as a volunteer, click “I am new to AYSO and want to volunteer” and follow the registration process.


     b. You will be assigned an AYSO ID number (as a volunteer).


2. Go to http://www.aysotraining.org/  and take the online Safe Haven training.


     a. Enter your AYSO ID and last name to log in.


     b. Under the “COURSE” tab, click on the AYSO Safe Haven course and take it.


3.  Register for a basic referee training course.  You may locate and register a course at www.eayso.org under "Section 2, Area B, Region 145".  Or email rra@millbraeayso.org for details.

INCIDENT REPORT FORM

The AYSO Incident Report Form is used whenever there is a personal injury, damaged property, or threats of or actual physical violence surrounding an AYSO game, practice, event or property.  The form should be prepared by the coach, AYSO Official, or AYSO Volunteer which may be a member of the regional staff such as the regional safety director, or by tournament or event staff members. Note:  This form should NOT be completed by a parent unless the parent is the coach.
