Online Sign-Up Instructions
Age Requirement: Players must be at least 6 years old on 8/31 and no older than 14.
1. Start your web browser and go to the website www.eayso.org (do not register via the REGISTER link at the top of the palosayso.org website). Disable any pop-up blockers.
2. If you never registered using eAYSO.org website before, you will need to create a user account first. Click on either of the two options:
a. My child is new to AYSO and wants to player soccer.
b. My child has played in AYSO but we never used eAYSO.
3. If you already have an eAYSO.org account, login using your email and password that you previously registered with. If you forgotten your email or password, click on one of the two links to retrieve the information. Call EAYSO SUPORT at 866-588-2976, option 2 if you don't see your child listed.
4. Use 733 if you are asked for a Region #. Region # 733 is our region.
5. You only need to field in the Required Fields. These fields are high-lighted in RED.
6. Please make sure we have a valid E-mail address and phone number. This is how the coaches will contact you or if we have any questions regarding your application.
7. For the uniform size, use the following guide-line. The same size will be used for both the top and bottom.
YXS= size 4-5
YS= size 5-6
YM=7-8
YL=10-12
AS=14-16
AM – AXL are also available.
8. Make sure to scroll down at every page and click NEXT (about 6 times).
9. If you completed everything correctly, the Player’s Registration Form will show on the screen. Scroll half way down until you see “I agree to use electronic signature”. Click on the checkbox next to this line to put a check mark and then type in your name. Scroll to the bottom and click on continue.
10. On the next screen, scroll all the way to the bottom and click on Submit.
11. Print out 1 copy of the player’s application and concussion form (3 pages total). Check to make sure it is digitally signed. Your name will be printed on the 1st page and there will be a note on the bottom of the 2nd page showing “Electronic Signature Process Record.”
12. Perform step 3-11 for every player.
13. Bring your completed Application to Registration night. If this is a new player, you must also bring a copy of their birth certificate for age verification or send a copy by email to [email protected] or text to 708-586-9268

15. All applications/documents need to be received by 5/15/16 to qualify for early registration. Otherwise, the late registration fee is $135* per player.
*Add $3.00 if paying by credit card. VISA, MasterCard, and Discover card accepted. Payment is accepted on-line at EAYSO.ORG.
NOTE: Every player must sign-up online. There will be no pre-printed forms or blank forms. Computers will be available on Registration nights. However, to speed up the registration process, we highly encourage you to sign-up ahead of time.
