2023 Player Forms

SPORTS PHYSICALS

For the 2023 Football Season, the MHSAA Representative Council and MHSAA SMAC is requiring all athletes to have an updated sports physical for the upcoming season before the first practice.

If athlete has not had a physical on or after April 15, 2023, then each athlete is required to provide a copy of a current sports physical by the first day of practice. Unfortunately, a well check visit is different than a sports physical, so we will need one that matches the form below.
