The forms on this page are required to be completed, signed and returned prior to the first practice.  These forms may be mailed into the WLC (see address under Club Information), turned in at In Person Registration, or any preseason event, such as the Meet the Coaches Night.

All Warrior Players will receive new uniforms for 2015.  The uniforms are shown below.  If you were unable to bring your player to the in-person registration event to be fitted, please use this sizing chart to help determine the correct uniform size to order. 

Please note: these jerseys will be customized with your player's name on the back, and so they cannot be returned nor refunded.  When sizing for a jersey, it is suggested that parents put shoulder pads on your son and see what size shirt fits him.  Also, please be aware that shorts must be ordered as well to complete the order.
