1) Scan the QR Code in the image above to download the PT Fitworks App.
2) Click "Sign-up" and Enter Requested Information to Register.
3) Fill out the Client form with DKS#, Check the required boxes and sign.
3) Use PT Fit Works App to Sign-In To The Health Club during your next visit using the barcode located in the "Onsite check in" tab.
