2022/23 REGISTRATION PROCESS

1.       From the Acceptance email you received please follow the instructions to accept your position and make your payment or arrange the payment plan.

a.       To pay by check select “Pay by Check” on the website and finalize the checkout process. Bring a check made out to “SP Soccer Academy” to registration.

b.       Credit card payments should be made through the website. Please select pay in full or the payment plan and finalize the checkout process. Please make sure you submit all process.

c.       PAYMENT PLANS ARE ONLY AVAILABLE ON THE WEBSITE BY CREDIT CARD.

2.       This Package contains all the forms required for registration. They include:

• Registration Checklist – Please bring to Registration Event with all Forms
  
  • U07-U19 Premier
  • U15-U19 Elite
  
  

3.       Please download and save forms to your computer, fill out forms, save again to computer, print, sign and bring them to the registration event or SP office located at 

a. These are fillable pdf forms. You can fill them out on your computer – use the TAB key to move from one field to the next – save to computer and print then sign them. Please make sure to fill out all the forms. The USYSA form requires THREE signatures.
