ALL injuries (or suspected injuries) and/or incidents need an Incident Report filled out.
AYSO Incident Report Form (with instructions)

This form is required when you are aware that a player sought medical attention for an injury and/or a concussion was suspected.
AYSO Participation Release Form (PLEASE NOTE...Michigan DOES require the Concussion Release form to be signed.)

If you have a player that walks home from practice/games or is picked up by someone other than their parent/guardian, then you need to have a copy of this form.
