ACKNOWLEDGEMENT

As a parent or guardian of the player applicant, I hereby give my consent for his/her participation in any and all activities of the Cedar Hill Youth Football & Cheer Organization (“CHYFCO”) during the upcoming CHYFCO football season. On behalf of the player applicant, I hereby assume any and all risks and hazards incidental to such participation including, but not limited to, transportation to and from any related sports activities. We hereby waive, release, indemnify, and agree to forever hold harmless CHYFCO, its officers and board members, and persons affiliated with CHYFCO transporting the player to and from sports activities, for any and all injuries to him/her. I further agree to abide by the guidelines in the CHYFCO Code of Conduct below, and will assume similar responsibility for any of my guests at CHYFCO-sponsored events. * PARTICIPANTS which play or cheer for the Cedar Hill Longhorns youth team for the 2015 season will not allowed to participate with other C.H.Y.F.C.O Extended / Independent Organizations without the written consent C.H.Y.F.C.O President in the following season. Participants must sit out a year.

MEDIA USE AND CONTENT

I hereby grant Cedar Hill Youth Football & Cheer Organization (hereinafter referred to as “CHYFCO”), the absolute right and permission to use my child’s likeness in a photograph, video, broadcast, publish or copyright and use pictures of my child in which he or she may be included in whole or in part, composite or retouched in character or form, without payment or any other consideration. I hereby irrevocably authorize CHYFCO to edit, alter copy, exhibit, publish or distribute photographs and videos of me or my child for informational, educational, promotional, or publicity purposes concerning CHYFCO and its services. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my child’s likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photographs or videos. I understand and agree that these photographs and videos will become the property of CHYFCO and will not be returned. I also understand the photographs and videos may be used without any further consent or authorization from me. I hereby hold harmless and agree to release and forever discharge CHYFCO, its officers, employees, volunteers, coaches, or agents, from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other person acting on behalf of myself or on behalf of my estate have by reason of Authorization.

PERMISSION & WAIVER

REFUND

I hear by acknowledge it is my responsibility to review the bylaws and playing rules. I understand there will be no refunds once the uniform is ordered. It is my responsibility to purchase padding to include shoulder pads and helmet.

SCHOLASTIC RESPONSIBILITY

I hereby stipulate that my child is scholastically fit to participate in CHYFCO activities.

EMERGENCY MEDICAL AUTHORIZATION

I hereby grant my permission for CHYFCO coach volunteers to provide or authorize any first aid as needed at their discretion, as long as they are not acting in good faith and without intentional misconduct. I further grant my permission for all emergency medical/dental treatment and /or first aid to be administered or authorized to my child/ participant by CHYFCO coach volunteers, including authorizing any medical treatment facility/hospital to administer emergency treatment for any illness/injury/accident resulting from participating in any and all CHYFCO activities. I agree to hold harmless and agree to release and forever discharge CHYFCO, its officers, employees, volunteers, coaches, or agents, from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other person acting on behalf of myself or on behalf of my estate have by reason of Authorization or Actions taken or not taken to administer first aid. I hereby stipulate that my child is medically/physically fit to participate in CHYFCO activities proof of school and/or grade attending.

TEAM, COACH, DIVISION, OR LEAGUE OF PARTICIPATION
As part of the registration process, specific teams, divisions, or coaches may be requested. CHYFCO will make every reasonable attempt to honor the requested team, coach, division, or league of participation. CHYFCO reserves the right to determine team composition, assign players to teams, make coaching changes, assign teams to divisions, and to determine which teams will participate in which leagues. Such changes are not grounds for registration fee refunds, regardless of the reason the change was made, or timing of the change. In addition, CHYFCO will not be held responsible for any costs or expenses incurred as a result of the change including added fees, sports physicals, travel, team gear, purchased protective equipment, spirit wear, sports fitness training, medications, or any other personal expenses not listed.

SUPPORTIVE CONDUCT, RULES AND REGULATIONS
I hereby understand and acknowledge that as a parent/guardian of a CHYFCO participant, it is my responsibility to be supportive and respectful of CHYFCO , the leadership and coaches, and my child, and to comply with all rules and regulations stipulated, adopted, or recognized by the CHYFCO organization and any League it participates. I further understand and acknowledge it is my responsibility to ensure my child is dressed appropriately for weather conditions, safely transported to and from all CHYFCO events on time, and at my own expense. Any unsupportive behavior, threats or public accusations made to CHYFCO, leadership, or coaches, or any non-compliance with any and all rules and regulations, may be cause for discipline and/or dismissal of my child (the participant), myself, and/or any spectators, or other persons affiliated with the named participant or myself.

IMMUNITY
I agree to hold harmless and agree to release and forever discharge CHYFCO , its officers, employees, volunteers, coaches, sponsors, or agents, from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other person acting on behalf of myself or on behalf of my estate have by reason of Authorization.

AUTHORITY TO LEGALLY ACT

I proclaim to have the legal authority to represent and register the named player/participant, and hereby stipulate that I have fully read, understand, and voluntarily agree to all of the terms, statements, and conditions contained in this waiver. In the event I share legal custody of the named player/participate, I proclaim to have fully collaborated with all other parents and guardians with shared legal custody and have their full permission to act. I have acquired unified consensus from all vested parties to register the named player under these terms, statements, and conditions.

CODE OF CONDUCT

This code applies to players, parents/guardians, coaches or anyone who attends a CHYFCO practice orgame:

• I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and

CHYFCO officials at every game or practice.

• I will place the emotional and physical well-being of my child ahead of any personal desire to win.

• I will insist that my child play in a safe and healthy environment.

• I will provide support for the coaches and officials working with my child to provide a positive, enjoyable experience for all.

• I will demand a drug, alcohol, and tobacco-free sports environment for my child and agree to assist by refraining from their use at all CHYFCO sports events, including practice.

• I will remember that the game is for the benefit of the children and not for the adults.

• I will do my best to make youth sports fun for my child.

• I will remain on the fan side of the field unless my child is hurt.

• I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability.

• I will promise to help my child enjoy the youth sports experience with my personal constraints by assisting with the coaching, being a respectful fan or whatever I am capable of doing.

• I will encourage my child’s coach to agree to and abide by the CHYFCO Rules of Conduct for Coaches.

• Parents are not allowed on the practice fields during practice or football field during games.

• This rule is in place to protect your children. The coaches have submitted to and passed a

background check. We do not want anyone who does not have a background check on file to be

responsible for our players.

Thank you for your support! Failure to agree to or abide by the CHYFCO Code of Conduct can lead to removal of the player/child from the game or practice facility or removal of parent/child from the league.
