You may also arrange to hand them to Maité Newby for Miami players at Edison High School or Martin Uranga for West Dade and Doral players at either Ruben Dario Park or Doral Legacy Park.  Please place forms in a envelope and clearly print your child's name

COMPETITIVE/TRAVEL - all competitive players are expected to submit the following paperwork, no exceptions.

Please submit 2 x passport pictures (new and returning players) writing players full name on the back, upload a copy of their birth certificate to your Blue Sombrero Account (here's how) and submit a copy to Juventus Academy  Administration at either of the emails listed above.
