Valley Youth United Soccer Clearance Checklist
The following checklist has been created to make sure that adult volunteers of Valley Youth United Soccer have the necessary clearances on file with the organization. If you have already obtained these clearances and have copies of them, you can follow the instructions for emailing them to us. Otherwise, please make sure to complete the following tasks:
☐ SAY Volunteer Application – download form (https://sports.bluesombrero.com/Default.aspx?tabid=431545&mid=455218&newskeyid=HN1&newsid=174368&ctl=newsdetail), complete, and sign
☐ Pennsylvania Child Abuse History Clearance – visit Child Welfare site (https://www.compass.state.pa.us/cwis/public/home), create individual login (if you don’t already have one), and complete process to obtain clearance
☐ Pennsylvania Criminal History Clearance – visit PATCH site (https://epatch.state.pa.us/Home.jsp) , click on New Record Check – Volunteers Only, complete process to obtain clearance.
☐ FBI Criminal History Check – ONLY REQUIRED IF YOU HAVE LIVED OUTSIDE OF PENNSYLVANIA IN THE LAST 10 YEARS – visit Identogo site (https://www.identogo.com/) and complete process.
☐ SAY Soccer Concussion Training - Follow directions at Concussion (saysoccer.org)
☐ Safesport Annual Training -Click on the following link and sign up for an account or sign into your existing account - https://safesporttrained.org?KeyName=JJFAQsQPWNRj01edWcPr
Once you have all the required clearances and the SAY Volunteer application filled out, please do the following:
- Attach copies of your clearances and volunteer application to an email.
- In the subject of the email, please put “Clearances”. This helps us to properly file and track the clearances you send in.
- Send email to [email protected].
We will process all clearances and send out a verification email to let you know that we have received them. Keep in mind that we need to have these on file for you to volunteer and interact with the kids.
