Parent Meeting Monday May 23rd at 6:00 pm.  This will be at Syracuse Junior High on the south field where check in was for tryouts.  You will turn in the following an then have a brief meeting with the coach. 

Please bring the following:

1 - $95 – if you already paid the Registration fee.  If you have not registered your child please go online to eayso.org register your player and bring that printed form with you along with the registration fee. 

2 - Birth certificate or copy of to verify birth date.  We will not be keeping this, just checking. 

3 - Completed the following form: extra registration form.docx.  If you can’t print it, you may fill one out that night, this will just speed up the process if you can.
