Registration

Below are the steps to follow AFTER you've been offered a position on a team.

Step 1 – Register online "Accept Now"


  • Login to Blue Sombrero
  • Follow the directions on the email you received about making the team.
  • If you have been contacted about being a “pool player” you will select that plan during checkout to ensure your dues are pro-rated correctly.
  • Upon completion of the registration and digitally signing the club policies, you will be prompted for payment. You may pay online or by check, but the initial registration fee must be paid before uniforms can be ordered or a player card is issued.
  • You can register multiple children at the same time using this system

Step 2 - Download and complete paperwork


Download and fill out the following forms..

General release waiver and videotape authorization
US Club Soccer Youth Registration Form and Medical release (pdf)

USYS Medical Release Form.pdf (FYSA)
Club Volunteer Waiver

Informed Consent about Concussions or Head Injuries

*** NOTE: If new to FC Dallas, please mail a copy of your players Birth Certificate ***

NOTE: The medical release form no longer has to be notarized.

Step 3 - Get sized for uniforms


Bring the completed paperwork and your child(ren) with you to registration. Sample uniforms will be available for sizing purposes.
