Refund Policy:
Requests for refunds will be accepted through July 1st.
Please submit your refund request to [email protected]
In your request, please include the following:
- Child's Full Name
- Amount Paid
- Payment Type (cash or check). If paid by check, please include check #.
AFTER July 1st, there will be NO REFUNDS!
On-Line Registration Process:
Go to www.eayso.org and follow the directions. Please be sure to e-sign your form.
A Birth Certificate is required if your child is a first time player in Moorpark AYSO.
We accept credit cards online or you can mail in a check with a copy of your application.
Your APPLICATION IS NOT COMPLETE until we receive payment and a copy of your application.

Open To:
Boys and Girls, ages 4 to 18 years old (birth dates between August 1st, 1997 and December 31st, 2012)
No previous soccer experience is needed. Boys and girls play on separate teams.

Registration Fee:
The registration fee is $145 for your 1st player, $135 for 2nd sibling and $125 each additional sibling in the same family.
In addition, see "Late Registration" below for more details.
Registration fees include uniform, pictures, supplemental insurance and Lot's of Fun!
Volunteers: Coach, Asst Coach, Team Parent, Referee, etc. must complete a volunteer application form every year and must register online at www.eayso.org. Please be sure to choose a position on your child's player form.

Teams
Coaches will receive their team information late July. Your coach should contact you within the first two weeks of August. If you haven't received a call by August 10th,
please follow up with the Division Director responsible for your child's age group. If you registered late and your child was placed on a waiting list, we will contact you
regarding your child's team assignment.
