Brea USGLL Photobook Order Form

Dear Parents:

Purchase your Brea USGLL 2014 Spring Photobook! The photobook includes the kid’s individual pictures, team pictures, activity
pictures and a CD for you to keep. The photobooks are limited, and books are ordered based on pre-purchased numbers. To guarantee your child receives a book by the end of the season, please purchase one NOW!

The cost of the photobook is $25.00. Deadline to order is May 30, 2014.

It’s complimentary for a family to put a remark and thought in the book for others to read and enjoy.

There will also be a section in the book for advertisement if you or anyone you know would like to sponsor the organization. Remember the fee is going toward the Brea USGLL organization for funding a scoreboard, goodies, supplies, water bottles and etc.

It is a tax write-off under USGLL EIN # 26-1902057. Writing a check to Brea USGLL, please give us a call at 714-350-7369, if you'd like additional information on our organization.
