Player's Name
Age
Parent's Name
Address to send refund
Reason for refund

Refunds will be awarded according to the following schedule:

Full refunds for requests received prior to July 1.
$35 for requests received July 1 to July 31.
NO Refunds will be given after  August 1.
NO exceptions
 
Please allow 2-3 weeks to process refund.
