Please Note: All Premier families must sign the 2015 Livermore Fusion Soccer Club program acceptance agreement and agree to keep to all the payments and costs associated with premier soccer.

*If all payments have been made by cash or check throughout the year, the final payment will be $300. If any payment has been made by Debit or Credit Card, the final payment for the program will be up to $349.

Reminder: A $50 late fee will be added to each payment that is not received by the scheduled dates. If payments are late by 30 days or more from due date your child's player card will be pulled and they will be unable to train or play in games.
