End of Season Summary Forms

The AYSA uses End of Season Summary forms to compare teams inside each division.  This information allows them to set up the most competitive division available and placing teams according to ability.  Each team in the LSA, U9 and up is required to complete an AYSA Season Summary Form.  You will need to list the teams you play against, location, final score and referee assignments.

You will be given a form in your coach’s packet at the start of each season.  The AYSA Season Summary form is also available from the link below.  Please print this form, complete it, and send it to the LSA  in two ways:

The LSA is charged a $10 fee for each team that does not complete a form.  Please complete this form throughout the season and get it back to LSA.
